Orthodontic Emergency Care

Most orthodontic problems are uncomfortable rather than dangerous, and most can be settled at home until we can see you. A poking wire, a loose bracket or a lost elastic tie are all common and none of them is a crisis. This page tells you what to do tonight, and which situations genuinely need attention right away.

Call your nearest Bright Orthodontics office during office hours and we will fit you in. Damaged appliances slow treatment down, so tell us even when nothing hurts.

Get Help Right Away If

These are the genuine emergencies, and they are rare.

  • A blow to the face, or a tooth knocked out or knocked loose. Contact your general dentist or seek urgent medical care immediately. Then call us.
  • A piece of an appliance is swallowed and you are coughing, choking or having any difficulty breathing. Call 911. If a small piece is swallowed without any breathing difficulty, call us and let us know.
  • Facial swelling, a fever, or a spreading infection. That needs a dentist or physician the same day, not an orthodontist.
  • Uncontrolled bleeding after an injury. Urgent medical care.

An orthodontic office is not the right place for any of these.

What to Do Right Now

A wire is poking your cheek or lip. Try tucking it out of the way with the blunt end of a toothbrush or the eraser end of a pencil. If it will not move, cover the end with orthodontic wax or a small piece of wet cotton, and call us. Do not cut the wire.

A wire has come loose from a bracket. Use the back of a spoon or a pencil eraser to gently push it back into place. If that is not possible, cover it with wax and call the office.

A bracket or band has come loose. If it is still on the wire, leave it there and wax over it if it rubs. If it has come off completely, keep it safe, bring it with you, and call for an appointment. Tell us about a loose band promptly, because decay can start underneath one quickly.

An elastic tie has come off. Not urgent. Note which tooth and mention it at your next visit, unless the wire has come out of the bracket too.

Your lips, cheeks or tongue are sore or rubbing. This is normal for the first week or two while the soft tissue toughens up. Wax over the offending bracket helps a great deal, and we will happily give you more. See braces and appliance care for the full first-weeks routine.

Everything aches after an adjustment. Also normal, and it usually settles in a few days. Dissolve one teaspoon of salt in eight ounces of lukewarm water, swish and gargle for a couple of minutes, and do not swallow it. An over-the-counter pain reliever helps if the rinse is not enough. Call us if the soreness is severe, or getting worse rather than better after several days.

A tooth feels loose. Teeth loosen slightly in order to move, so this is expected. It settles once the tooth reaches its new position.

Your retainer is broken, lost or no longer fits. Do not force it back in. Call us. See orthodontic retainers.

What Can Wait Until Morning

Nearly all of it. If wax and a salt water rinse make you comfortable, the repair can almost always wait for office hours.

Call your nearest office when we open and describe what happened and when. That lets us book the right length of appointment. Bring any piece that came off with you.

Preventing the Next One

Most breakages trace back to the same short list: ice, hard rolls, nuts, popcorn kernels, sticky candy, and chewing on pen lids or fingernails. Call Us, Even If It Seems Minor

Nobody has ever annoyed us by calling about a wire.

Emergency Questions

Can I cut a poking wire myself?
Please do not. Cutting risks a sharp edge, a swallowed fragment and a longer repair. Wax it, and call us. If you are in real difficulty and far from any of our offices, call us first and let us talk you through it.

Can I see a different orthodontist if I am away from home?
Yes, for an urgent repair. Most practices will help a traveling patient with a poking wire or a loose bracket. Call us too, so we know what was done.

Can the emergency room help with a broken bracket?
Not usefully. An emergency room handles trauma, infection, swelling and bleeding. For a bracket or a wire, save the trip and call us.

How long can I leave a broken bracket before it matters?
A few days will not undo your treatment, but weeks will. While a bracket is off, that tooth is not being worked on while the rest of the arch keeps moving. Tell us as soon as you notice.
